Over 3,500 Cases Reported Nationally Since January 2026
South Africa has recorded 2,476 laboratory-confirmed measles cases between December 29, 2025 and June 21, 2026, with most cases in children aged 1 to 14 years. More than 120 additional cases were reported in July 2026 alone. The outbreak is now active across all provinces, with Western Cape and Free State among the hardest hit. Vaccination coverage has declined from 95% to approximately 70 to 80%, driven by misinformation and vaccine shortages.
Notably, nearly 18% of confirmed cases have occurred in people over 15 years old, suggesting an immunity gap that extends beyond children into older teenagers and adults. This matters for travelers of all ages.
Symptoms to watch for
Symptoms typically appear 7 to 14 days after exposure. Watch for:
- High fever, cough, runny nose, and red watery eyes
- A red blotchy rash beginning on the face and spreading downward
If you develop these symptoms after returning from South Africa, call your doctor before going in so they can prepare.
How to protect yourself
- All persons born in 1957 or later without a history of measles or two documented MMR doses should complete a lifetime total of two doses, spaced at least 28 days apart
- Infants aged 6 to 11 months should receive one MMR dose before travel
